Procedures and Requirements for New Enrollees and Transferees

Window 1
Step 1: Submit the following:
- Form 138/Report Card (original and photocopy)
- Birth and Baptismal certificates (original and photocopy)
- Certificate of God Moral and Character from the principal of previous years
- Two (2) 1x1 ID pictures
Window 2
Step 2: Fill up the registration form
Step 3: Submit for Assessment.
Window 3
Step 4: Pay admission fee to the cashier
Step 5: Take the entrance exam at the Guidance center
Step 6: Get the result of the test the following day or as advise
Step 7: Interview by the Academic Coordinator and the Principal
Window 4
Step 8: Sectioning
Step 9: Keep student's copy and registration form and present it to the class adviser on first day of classess
Procedures and Requirements for Old Students
Window 1
Step 1: Submit the following:
- Form 138
- Properly accomplished registration form
- Clearance from the Prefect of Discipline
- Proceed to the Principal's office for approval and signature
Window 2
Step 2: Submit form for assessment
Window 3
Step 3: Pay admission fee to the cashier
Window 4
Step 4: Sectioning
Step 5: Keep student's copy and registration form and present it to the class adviser on the first day of classes
